Community-Based Supports
Community-based services include but are not limited to: providing one-on-one training and supervision of individuals as they learn and perform tasks included in basic self-care and activities of daily living. These support services allow individuals to develop skills and participate in community-based activities. They are thoughtfully designed to help individuals with disabilities live as independently as possible while participating fully in their communities.
Individual supports
Individual supports is assistance provided to individuals with disabilities to help them live as independently as possible and participate in their community. A Bridge Healthcare Services helps individuals achieve the outcomes of increased independence, productivity, enhanced family functioning and inclusion in the community.
Pre-vocational
Prevocational services are designed to help individuals develop the skills and knowledge needed to participate in paid or unpaid work. Prevocational services are an important part of an individual's transition from school to work and can be used to help individuals who are returning to the workforce after a period of absence.
Community inclusion
Community inclusion services are programs and activities that help individuals participate fully in their communities and play an active role in society. Community inclusion programs at A Bridge Healthcare Services are designed to help individuals feel fully included and become valued members of their communities.
Respite
Respite care is short term care provided to individuals, allowing primary caregivers to take a break from caregiving responsibilities.
A Bridge Healthcare Services provides respite care in several settings including: in-home, at a respite care center, or in a residential facility. We provide respite care is provided on a scheduled or as-needed basis.

Natural supports
Natural supports is assistance provided by individuals who are already a part of an individual's life, such as family members, friends, and members of the community.
